Improving Root Cause Analyses
February 13, 2018 8:00am - 4:00pmThe Perioperative Quality Improvement Committee is excited to host a one-day conference entitled; "Improving Root Cause Analyses and Actions to Prevent Harm" Facilitators James P. Bagian, MD, PE, Director of the Center for Healthcare Engineering and Patient Safety (CHEPS) at the University of Michigan; and Joseph M. DeRosier, PE, CSP, Program Manager for CHEPS will teach participants how to implement a standardized process that identifies root causes and contributing factors as well as effective and sustainable corrective actions to reduce or eliminate future patient harm. This process, known as the RCA2, published by the National Patient Safety Foundation,is endorsed by the Joint Commission and healthcare organizations around the country.
